History

A 25 year old male resents with a painful right eye with reduced vision. No trauma to eye.

Examination shows vitreous opacities.

A diagnostic vitrectomy is done.

Questions

  1. Figures 1 and 2 are sections of the vitrectomy specimen. What are the ovoid structures indicated by the arrows?
  2. What do the silver stains show (Figs 3 and 4)?
  3. Diagnosis?

  1. These are granulomas.
  2. Silver stain showing ‘bangers and beans’ hyphae and spores.
  3. Candida endogenous endophthalmitis (culture positive). In this case, only one granuloma contained the candida, after examining many levels.
